Eligibility Requirements:

• $0 Account Balance: All outstanding fees must be paid in full.
   
• Driver’s License: Upload a clear photo or scan.
   
• Previous Fob Serial Number: (If you have one.)
   
• Complete Application: All fields in the form are required.
   

Important Details:

• CAM will contact you once your application has been processed.
   
• Complimentary Device: One per household, free if you have submitted your previous device number.
   
• Replacement Fee: $25 for lost or stollen devices.
